AI Appointment Reminder Calls: Rules and Setup

Pew Research found that most Americans don't answer calls from unknown numbers, which limits reminder calls. Calling from the business number the customer already knows helps. Common pattern: text first, and use an AI call for customers without a mobile number, for high-value appointments, or when a text gets no reply.
Consent
Under the TCPA, AI voices count as artificial voices (FCC, 2024), so reminder calls need prior express consent. A customer who gives their number to book an appointment has generally consented to calls about that appointment. Keep the call strictly informational:
OK: "Calling to confirm your appointment Thursday at 2 p.m. with Dr. Lin."
Not OK on a reminder: "...and while I have you, we're running a whitening special."

Timing
- One to two business days before gives time to fill a canceled slot.
- During business hours, within federal and state calling windows. See AI calling hours rules.
- One attempt per appointment, plus at most one retry. Repeated calls annoy people and can run into state frequency caps.
What the agent should say
Structure:
- Identify: business name, agent name, that it's an AI assistant.
- Purpose: confirming an appointment.
- Details: day and time (and person or location if useful).
- Ask: "Will you be able to make it?"
- Handle the answer (confirm, reschedule request, cancel).
- Close with any preparation instructions.

If they need to reschedule, ask for two or three times that work
and schedule a callback so the team can confirm the new time.
If they want to cancel, note it and tell them the team will
update the schedule. Don't mention promotions or other services.
If they ask to stop receiving calls, confirm and end the call.
Handling reschedules
Unless the AI is connected to your live calendar, it shouldn't promise a new time. It should collect preferences and hand them to your team:
"No problem. What days and times would work better next week?" "Tuesday morning or Wednesday after 3." "Got it. I'll pass that to the front desk and they'll confirm a new time with you today."

Voicemail
Most reminder calls will reach voicemail. A good AI voicemail:
- Names the business.
- Says it's an appointment reminder.
- Asks the person to call back if they need to change anything.
- Stays under 20 seconds.
For healthcare and other sensitive services, leave out details like the type of appointment or the provider's specialty. "This is a reminder from Lakeview" is enough.
